Registered General Health Nurse

We are seeking a skilled and experienced Registered General Health Nurse to join our team as a Band 5 Nurse. The successful candidate will be responsible for supporting and advising wards in the timely planning and execution of patient discharge.
Key Responsibilities- Provide comprehensive information to patients and families regarding discharge planning and management.
- Collaborate with ward teams to identify patients with complex needs and develop effective discharge plans.
- Ensure safe and efficient transfers of care from acute settings to community or home settings.
- Support the implementation of the SAFER methodology to enhance patient care and outcomes.
- Participate in daily board rounds to ensure all patients have a meaningful estimated day of discharge.
- Capture and record data accurately on the Frontier system.
- Current NMC registration.
- Pre-registration evidence in direct nursing care.
- Experience in providing holistic nursing care and developing nursing skills.
- Previous clinical experience and knowledge of acute hospital functions.
- Experience in coordinating and liaising with simple and complex discharges.
- Degree in Nursing.
- Evidence of post-registration study and training, e.g. POVA, Child Protection, and Intermediate Life Support.
- Clear understanding of clinical governance framework and evidence-based practice.
- Welsh language skills (Level 1).
