About the Role

We are seeking a skilled and experienced Registered General Health Nurse to join our team as a Band 5 Nurse. The successful candidate will be responsible for supporting and advising wards in the timely planning and execution of patient discharge.

Key Responsibilities
  • Provide comprehensive information to patients and families regarding discharge planning and coordination.
  • Manage patient and family expectations to minimize hospital stay duration.
  • Collaborate with acute ward teams to identify patients with complex needs and develop effective discharge plans.
  • Support the implementation of the SAFER methodology to ensure safe and effective transfers of care.
  • Participate in daily board rounds to ensure all patients have a meaningful estimated day of discharge.
  • Capture and record data on the Frontier system in real-time.
Requirements
  • Current NMC registration.
  • Pre-registration evidence in direct nursing care.
  • Experience in providing holistic nursing care and developing nursing skills.
  • Previous clinical experience and knowledge of acute hospital functions.
  • Experience in coordinating and liaising with simple and complex discharges.
Desirable Qualifications and Skills
  • Degree in Nursing.
  • Evidence of post-registration study and training, e.g. POVA, Child Protection, and Intermediate Life Support.
  • Clear understanding of clinical governance framework and evidence-based practice.
  • Welsh language skills (Level 1).
