We are seeking a highly skilled and experienced Minor Illness and Practice Nursing Specialist to join our team at Amwell Surgery. As a key member of our primary care team, you will be responsible for providing high-quality nursing care to patients with minor illnesses and chronic conditions.
Main Responsibilities:
- Provide comprehensive nursing care to patients with minor illnesses and chronic conditions, including wound care, chronic disease management, and health promotion.
- Work collaboratively with GPs and other healthcare professionals to provide holistic care to patients.
- Develop and implement individualized care plans to meet the unique needs of patients.
- Conduct health assessments, take medical histories, and perform physical examinations as needed.
- Order and interpret diagnostic tests, such as blood work and urinalysis.
- Prescribe medications and treatments as authorized by the GP.
- Provide education and support to patients and their families on health-related topics.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Registered Nurse (Nursing and Midwifery Council) qualification.
- Independent prescriber qualification.
- Postgraduate Diploma or degree in Nursing (Nurse Practitioner).
- Experience in primary care and chronic disease management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
