We are currently seeking a Primary Care Nurse Practitioner to join our team. This is an exciting opportunity for an experienced nurse to work alongside our practice nurses and doctors to deliver high-quality patient care. The successful candidate will be responsible for managing chronic diseases, performing routine immunisations, and providing general health advice to patients.

Responsibilities

  • Manage chronic diseases such as diabetes, asthma, COPD, and hypertension
  • Perform routine immunisations, including child immunisations
  • Provide general health advice to patients
  • Treat minor illnesses and injuries
  • Refer patients to specialists when necessary
  • Maintain accurate and contemporaneous records
  • Participate in practice meetings and contribute to team discussions

Requirements

  • Registered Nurse (Nursing and Midwifery Council)
  • Independent Prescriber
  • Postgraduate Diploma or Degree in Nursing (Nurse Practitioner)
  • Experience in primary care
  • Knowledge of chronic disease management

Salary and Benefits
