Job Summary:
We are seeking an experienced Social Worker to join our team at Community Counseling of Bristol County. As a member of our multidisciplinary team, you will provide intensive care coordination services to children aged birth through 21 with mental health needs.

About Us:
Community Counseling of Bristol County is a non-profit community mental health center serving 50 communities in Massachusetts. We provide inpatient, outpatient, and urgent care services to adults and children overcoming depression, anxiety, mood disorders, substance abuse, and many other conditions hindering personal fulfillment.

Responsibilities:
• Engage youth, caregivers, providers, and supports in establishing Care Planning Teams (CPTs) to address youth's identified goals
• Partner with youth, caregivers, and team members in the completion of comprehensive assessments and treatment planning
• Collaborate with youth and caregivers in promoting the involvement of natural and community-based supports in planning for each youth
• Work collaboratively with providers such as educational systems, Department of Children and Families, clinical supports, and other stakeholders to support youth in achieving their identified treatment goals
• Provide services in the community
• Complete documentation in line with agency and insurance requirements

Requirements:
• Master's degree in Social Work or related field, or
• Bachelor's degree in Social Work or related field and 3 years' experience working within the child/family system
• Must have a dependable car, valid driver's license, and proof of insurance
• Experience in navigating any of the child/family-serving systems and experience advocating for family members who are involved with behavioral health systems
• Experience in Wraparound is preferred
