About the Role

We are seeking a highly motivated and experienced Social Worker to join our Adult Care team in Rochdale. As a Social Worker, you will play a vital role in supporting vulnerable adults and their families, providing expert advice and guidance to ensure the best possible outcomes.

Key Responsibilities
  • Conduct assessments and develop care plans to meet the needs of service users
  • Work collaboratively with other professionals to ensure seamless delivery of services
  • Provide expert advice and guidance to service users, families, and carers
  • Develop and maintain effective relationships with service users, families, and carers
  • Contribute to the development and implementation of policies and procedures
