About the Role

We are seeking a highly motivated and experienced Social Worker to join our Adult Care team in Rochdale. As a Social Worker, you will play a vital role in supporting vulnerable adults and their families, providing expert advice and guidance to ensure the best possible outcomes.

The successful candidate will have a strong understanding of adult social care principles and practices, with excellent communication and interpersonal skills. You will be able to work effectively with a range of stakeholders, including service users, carers, and other professionals, to deliver high-quality care and support.

Key Responsibilities
  • Assess and identify the needs of adult service users, developing and implementing effective care plans to meet their needs.
  • Work collaboratively with other professionals, including healthcare and housing teams, to ensure seamless care and support.
  • Provide expert advice and guidance to service users, carers, and other professionals, ensuring they have access to the information and resources they need.
  • Contribute to the development and implementation of adult care policies and procedures, ensuring they are effective and meet the needs of service users.
  • Participate in multi-agency meetings and forums, working collaboratively with other professionals to improve adult care services.

Requirements

To be successful in this role, you will need:

  • A degree in Social Work or a related field.
  • Experience of working in adult social care, preferably in a similar role.
  • Excellent communication and interpersonal skills.
  • A strong understanding of adult social care principles and practices.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple deadlines.
