Key Responsibilities- Medication Management: Provide expert advice on medication management, including prescribing, monitoring, and reviewing medication regimens for patients with complex needs.
- Clinical Audits: Conduct clinical audits to identify areas for improvement in prescribing practices and implement changes to optimize patient care.
- Education and Training: Provide education and training to healthcare professionals on therapeutics and medicines optimization, ensuring they have the necessary skills to provide high-quality patient care.
- Collaboration and Communication: Work closely with multidisciplinary teams, including GPs, nurses, and other healthcare professionals, to ensure seamless care coordination and effective communication.
- Quality Improvement: Contribute to quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care and outcomes.
- Qualifications: Master's degree or equivalent in Pharmacy, with a strong background in clinical pharmacy practice.
- Registration: Registration with the General Pharmaceutical Council (GPhC) and independent prescribing qualification.
- Experience: Minimum of 2 years postgraduate clinical experience, with a strong track record of working in a community setting and collaborating with multidisciplinary teams.
