Key Responsibilities

  • Support the Intensive Support Team in providing assessments, care, and treatment for individuals with learning disabilities, mental health issues, and challenging behaviors in the community.
  • Use your skills to work with individuals with learning disabilities who are experiencing a crisis to maintain, promote, and improve their health.
  • Provide advice and consultation to colleagues supporting autistic adults who do not have learning disabilities.
  • Facilitate access to health services through collaboration and partnership with primary and secondary health, and other relevant services.
  • Develop working relationships with clients, provider organizations, GPs, and local community resources.
  • Support the delivery of generic health tasks such as the completion of Hospital Passports, Health Action Plans, monitoring of mental health, and behaviors that may challenge.
  • Prepare clients and carers for health appointments to enhance their understanding and enable them to actively participate in their own healthcare.
  • Contribute to shaping local health services to promote an inclusive service for people with learning disabilities.
