Community Rehabilitation Specialist

We are seeking a highly skilled Reablement Therapist to join our Community Independence Service (CIS) team in the Royal Borough of Kensington and Chelsea. As an Occupational Therapist, you will be responsible for assessing individuals' capabilities and developing personalized plans to support their rehabilitation and reablement needs.
Key responsibilities include:
- Carrying out OT assessments for disabled people and their carers
- Developing and implementing individualized reablement plans
- Working collaboratively with other professionals to ensure seamless care delivery
A competitive salary of £41,967 - £50,088 per annum reflects the importance of this role, which involves regular liaison with electronic case management systems and completion of Care Act 2014 assessments.
