We are recruiting a Social Care Practitioner to provide high-quality support to individuals in our Midlothian services. As a Social Care Practitioner, you will be responsible for developing and implementing personalized support plans, ensuring that each individual receives the care and support they need to achieve their goals.

You will work collaboratively with our multidisciplinary team to identify the unique needs and aspirations of each client, and develop tailored support strategies to meet those needs. Your primary focus will be on promoting independence, autonomy, and well-being, while also providing emotional support and companionship.
