Job Title: Community Treatment Team Practitioner

About the Role

The Community Treatment Team Practitioner will work as part of a multidisciplinary team to assess and manage patients with complex needs. You will be responsible for providing professional and clinical support to staff working within the CTT and collaborative services such as Acute hospital trust, GPs, NHS 111, and Ambulance Services.

Key Responsibilities
  • Manage a specific caseload of clients, using the nursing process of assessment, planning, implementation, and evaluation.
  • Provide professional and clinical support to staff working within the CTT and collaborative services.
  • Participate in the development of a high-quality service clinical governance framework.
  • Assist with the day-to-day management of the CTT and coordination of workload and development of the team.
  • Participate in the implementation of NELFT & NEL service developments impacting on Community Services.
  • Establish and maintain effective communication and relationships across all disciplines and services.
  • Act as an autonomous practitioner to assess, plan, implement, review, and evaluate care, including complex care assessments.
Requirements
  • First-level Registered Nurse currently registered with the Nursing & Midwifery Council/BSc (Hons) Paramedic Science/Dip HE in Paramedic Science registered with the HCPC.
  • Practice educator or other teaching qualification or equivalent experience.
  • Physical Clinical Assessment Skills Course or willingness to undertake the course.
  • Qualification in minor illness Management.
  • Experience of working in Primary Care/Accident & Emergency or/and Qualifications in Community and/or Accident & Emergency Nursing.
  • Non-Medical Prescriber (NMP) or willingness to undertake the course.
