Job Summary

Care New England Health System is seeking a skilled and compassionate Nurse (Part Time) to join our team. As a Home Health Clinician, you will be responsible for managing a caseload of patients, coordinating interdisciplinary care, and monitoring treatment to achieve positive therapeutic outcomes.

Key Responsibilities
  1. Manage a caseload of patients from admission to transition into the community, involving interdisciplinary support and referrals when appropriate.
  2. Coordinate interdisciplinary care, monitor and adjust treatment to achieve positive therapeutic outcomes.
  3. Identify opportunities for process improvement and implement changes as needed.
  4. Work collaboratively with the Team/Program Manager to ensure effective care delivery.
Requirements
  1. Equivalent education required for RI Registered Nurse licensure; Bachelor's Degree (min) in relevant discipline strongly preferred.
  2. Valid RI Registered Nurse license required.
  3. A minimum of one (1) year progressive clinical experience required. Previous experience in an acute care or Home health care environment setting preferred.
  4. Valid CPR certification required.
  5. Valid driver's license and automobile liability insurance are required.
  6. Excellent communication and interpersonal skills, with the ability to read, write, and speak English fluently.
  7. Ability to define problems, collect data, establish facts, and draw valid conclusions.
