Care New England Health System is seeking a skilled and compassionate Nurse (Part Time) to join our team. As a Home Health Clinician, you will be responsible for managing a caseload of patients, coordinating interdisciplinary care, and monitoring treatment to achieve positive therapeutic outcomes.
Key Responsibilities- Manage a caseload of patients from admission to transition into the community, involving interdisciplinary support and referrals when appropriate.
- Coordinate interdisciplinary care, monitor and adjust treatment to achieve positive therapeutic outcomes.
- Identify opportunities for process improvement and implement changes as needed.
- Work collaboratively with the Team/Program Manager to ensure effective care delivery.
- Equivalent education required for RI Registered Nurse licensure; Bachelor's Degree (min) in relevant discipline strongly preferred.
- Valid RI Registered Nurse license required.
- A minimum of one (1) year progressive clinical experience required. Previous experience in an acute care or Home health care environment setting preferred.
- Valid CPR certification required.
- Valid driver's license and automobile liability insurance are required.
- Excellent communication and interpersonal skills, with the ability to read, write, and speak English fluently.
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
