Job Summary: We are seeking a highly skilled Care Coordinator to join our team at Adult Social Care. The successful candidate will be responsible for delivering individualized support programs to foster children and their carers, while also facilitating training sessions to enhance the skills of foster carers.

Key Responsibilities:

  • Case Management: Provide guidance and practical assistance to foster carers to ensure the well-being of the children in their care.
  • Direct Support: Deliver tailored support programs to meet the unique needs of each child and young person, and their carers.
  • Training & Development: Collaborate with colleagues to co-run support groups and facilitate training sessions for foster carers to improve their capabilities.
  • Multi-agency Collaboration: Work closely with professionals from various sectors to provide comprehensive support to the welfare of children and young people.
  • Fostering Recruitment: Assist in recruiting foster carers and providing information about the role of fostering to prospective applicants.


Requirements:

  • Qualifications: A relevant qualification such as NNEB, BTEC in Nursery Nursing, NVQ Level 3 in Childcare and Education, or equivalent qualifications in Children and Young People's Health & Social Care is required.
  • Experience: Demonstrated experience working with children, young people, and families in a team setting and collaborating with external agencies is essential.
  • Skills: Strong record-keeping, report-writing, IT skills, and the ability to assess needs and plan individualized programs are crucial.
  • Knowledge: Understanding of Fostering Regulations, National Minimum Standards, and Care Planning Regulations is necessary.
