Job Title: Adult Therapist

Central and North West London NHS Foundation Trust is seeking a skilled Adult Therapist to join our team.

Job Summary

We are looking for a highly motivated and experienced Adult Therapist to work with our Multi-Disciplinary Team to provide comprehensive assessments and treatment plans for patients with diverse or complex presentations.

Key Responsibilities
  • Undertake comprehensive assessments of patients, including those with dementia and confusion, physical frailty and disability, and other complex presentations.
  • Work with the acute Multi-Disciplinary Team and Transfer of Care Hubs to promote efficient and effective discharge from hospital to the patient's own home.
  • Implement and evaluate treatment plans, coordinate interventions, and provide peer support for members of the MDT.
  • Liaise with health and adult social care, working in partnership with primary and secondary care, social prescribers, and other external agencies to ensure delivery of a high-quality service.
  • Communicate effectively with patients, carers, and allied agencies to maximize discharge and recovery potential.
  • Work collaboratively with primary and secondary health care services, third sector services, and social prescribing, education, and social services to ensure seamless and high-quality service delivery.
  • Provide a 2-hour urgent response for patients in crisis, utilizing advanced skills in delivering therapy interventions.
  • Carry out risk assessments in line with Trust/National Protocol.
Requirements
  • Diploma/degree in Physiotherapy or Occupational Therapy.
  • Membership of the Chartered Society of Physiotherapy or RCOT.
  • Attendance at recent postgraduate courses.
  • Experience of community work, working with patients with dementia and confusion, physical frailty and disability, and student and therapy assistant supervision.
  • Understanding of the legal responsibilities of the profession.
