Job Title: Adult Therapist

We are seeking a highly skilled and compassionate Adult Therapist to join our team at Central and North West London NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and support.

About the Role

The successful candidate will be responsible for assessing patients with diverse or complex presentations, working collaboratively with the acute Multi-Disciplinary Team and Transfer of Care Hubs to promote efficient discharge from hospital to the patient's own home. You will also be responsible for managing patients' care pathways through the CIS services, implementing and evaluating treatment plans, and coordinating interventions within Home First & Urgent Community Response.

Main Responsibilities
  • Undertake comprehensive assessments of patients, including those with diverse or complex presentations/multi pathologies.
  • Work with the acute Multi-Disciplinary Team & Transfer of Care Hubs to promote an efficient, effective discharge from hospital to the patient's own home.
  • Manage patients' care pathways through the CIS services.
  • Implement and evaluate treatment plans, and coordinate interventions within Home First & Urgent Community Response.
  • Liaise with health, and adult social care, working in partnership with primary and secondary care, social prescribers, and other external agencies to ensure the delivery of a high-quality service.
  • Provide a specialist service to patients in their own home/usual residence.
  • Communicate effectively with patients and carers and allied agencies to maximise discharge and recovery potential and to ensure understanding of condition.
Requirements
  • Registration with HCPC.
  • Membership of the Chartered Society of Physiotherapy OR Membership of the RCOT.
  • Attendance at recent postgraduate courses.
  • Evidence of CPD maintained in a portfolio.
  • Involvement in CSP Special Clinical Interest Groups OR RCOT Special Clinical Interest Groups.
Knowledge & Experience
  • Must be able to provide evidence (through appraisals, competencies and reflection from post graduate study) to demonstrate development of specialist knowledge.
  • Experience of community work.
  • Experience with patients with dementia and confusion.
  • Experience with patients with physical frailty and disability.
  • Knowledge of validated assessment tools and outcome measures.
  • Experience of student and therapy assistant supervision.
  • Specialist knowledge of broad range of conditions.
  • Experience of working as part of a therapy team and/or multidisciplinary team.
  • Extensive post qualified experience in profession, to include acute experience.
  • Experience in audit or measuring clinical effectiveness.
  • Evidence of ability to quickly gain and maintain credibility across the Trust and externally, with a wide range of professions.
  • Remain updated with professional and evidence-based practice and new research.
  • Understand the legal responsibilities of the profession.
  • To be competent in performing complex assessments and treatments with patients with a variety of conditions.
  • Regularly undertakes moderate physical effort for multiple periods on a daily basis, whilst carrying out concurrent activities.
