**Job Summary**

Creative Support is a leading provider of person-centred care and support services for older adults, individuals with learning disabilities, mental health needs, and autism spectrum conditions.

**Job Role:**

We are seeking a Relief Support Worker to join our Mental Health Care Service in Stretford. As a Relief Support Worker, you will provide practical and emotional support to service users in line with their Personal Support Plans.

**Key Responsibilities:**

  • Provide person-centred support to service users, promoting independence and confidence.
  • Identify and access opportunities for service users to gain independence and confidence.
  • Work with service users, families, and involved professionals to provide a coordinated service that maximises outcomes for service users.
  • Collaborate with multidisciplinary teams to ensure a consistent and effective service delivery.

**Requirements:**

  • Experience of working with individuals with mental health needs.
  • A warm and empathetic personality, with the ability to work in a person-centred way.
  • Flexibility to work varied shifts, including evenings, weekends, sleep-ins, and bank holidays.
