About the Role
Intrahealth Ltd is seeking a skilled Care Coordinator to join our team at the West Cornforth Practice. As a Care Coordinator, you will play a key role in proactively identifying and working with people who engage with the practice irregularly and those who fail to engage with smear programmes, child vaccinations, etc.
Main Responsibilities
- Proactively identify and work with people who engage with the practice irregularly and those who fail to engage with smear programmes, child vaccinations, etc.
- Develop relationships with hard-to-reach groups and understand how the practice can adapt to better understand and meet their needs.
- Work closely with GPs and practice teams to ensure that appropriate support is made available to people.
- Signpost patients to the most suitable external organisations who could support with issues that were less directly health-related, such as citizens advice, wellbeing for life, exercise referral, community groups, etc.
- Attend multidisciplinary team meetings within the practice to ensure appropriate follow-up for patients occurs.

We operate NHS services across the North East, North West, and Yorkshire.
