About the Role

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Intrahealth Ltd. As a Care Coordinator, you will play a vital role in proactively identifying and working with individuals who engage with our practice irregularly and those who fail to engage with smear programs, child vaccinations, and other essential health services.

Key Responsibilities
  • Proactively identify and work with individuals who engage with our practice irregularly and those who fail to engage with smear programs, child vaccinations, and other essential health services.
  • Develop relationships with hard-to-reach groups and understand how our practice can adapt to better meet their needs.
  • Work closely with GPs and practice teams to ensure that appropriate support is made available to individuals.
  • Signpost patients to external organizations that can provide support with issues that are less directly health-related, such as citizens' advice, wellbeing for life, exercise referral, and community groups.
  • Attend multidisciplinary team meetings to ensure appropriate follow-up for patients occurs.

Person Specification
  • Demonstrable commitment to professional and personal development.
