About the Role

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at the Heart of Lincoln Medical Group (HLMG). As a key member of our multidisciplinary team, you will be responsible for delivering high-quality patient care and working collaboratively with our healthcare professionals to meet the needs of our patients.

Key Responsibilities
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice and guidance.
  • Implement and evaluate individualized treatment plans for chronic disease patients, utilizing your expertise and knowledge to optimize patient outcomes.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health and well-being.
  • Provide advanced, specialist nursing care to patients as required, adhering to clinical guidelines and best practices.
  • Request pathology services as necessary and process and interpret test results to inform patient care.
  • Deliver daily clinics, providing patient care and referring patients to secondary or specialist care as required.
  • Maintain accurate clinical records, ensuring compliance with legislation and regulatory requirements.
  • Ensure read codes are used effectively and maintain chronic disease registers in accordance with QOF guidelines.
  • Chaperone patients as necessary and prioritize health issues, intervening appropriately to ensure optimal patient outcomes.
  • Support the team in dealing with clinical emergencies and recognize, assess, and refer patients presenting with mental health needs.
  • Act as an extended and supplementary prescriber, adhering to guidance and best practices.
  • Support patients in the use of their prescribed medications and review annually as required.
  • Contribute to practice targets and comply with local and regional guidance.
  • Liaise with external services and agencies to ensure patients receive comprehensive support.
  • Delegate clinical responsibilities appropriately, ensuring safe practice and task delegation within the scope of practice.
  • Support the clinical team with safeguarding matters, adhering to local and national policies.
  • Understand practice and local policies for substance abuse and addictive behavior, referring patients as necessary.
Person Specification
  • Registered Healthcare Professional Nurse or Paramedic.
  • Qualified Independent Prescriber V300.
  • Qualified Triage Nurse/Clinician.
  • Minor Illness Qualification.
  • Postgraduate diploma or degree (Advanced Practice Qualification) or working towards this.
  • Experience of working in a primary care environment.
  • Experience of working as a practice nurse or community nurse.
  • Experience of chronic disease management.
  • Experience of prescribing and undertaking medication reviews.
