This primarily involves support using a person-centred approach. Our Support Workers measure outcomes specifically to an individual's needs and evidence using our person-centred outcome-measuring tool.

Job Description

We are seeking a Support Worker to join our team in Altrincham, Cheshire. As a Support Worker, you will be responsible for providing person-centred care and support to adults with mental health needs. You will work as part of a team to deliver high-quality care and support, ensuring that each individual receives the best possible outcomes.

The successful candidate will have experience in a care environment, with good communication skills and the ability to work independently and as part of a team. You will be passionate about making a positive difference to someone else's life and have a commitment to delivering person-centred care.

Key Responsibilities
  • Provide person-centred care and support to adults with mental health needs
  • Work as part of a team to deliver high-quality care and support
  • Measure outcomes specifically to an individual's needs and evidence using our person-centred outcome-measuring tool
  • Develop and maintain relationships with individuals, families, and carers
