Dementia Care Manager

About the Role:
Hallmark Care Homes is seeking a dedicated Dementia Care Manager to lead our dementia community and drive our relationship-centred care strategy.
As a key member of our team, you will be responsible for managing a committed and loyal caring team, ensuring our community meets the highest standards of care and quality.
You will work closely with our Head of Relationship Centred Care and Dementia Relationship Practitioner to deliver exceptional evidence-based care and develop strong links with local community groups involved in dementia care.
With a passion for dementia care and a commitment to excellence, you will be the smiling face and go-to person for our residential and nursing communities, providing expert knowledge and support to families and friends of people living with dementia.
Key responsibilities will include ensuring the dementia community meets all CQC requirements, Hallmark's quality and compliance framework, and developing solid relationship bonds with the General Manager and other internal Heads of Department.
