As a key member of our multidisciplinary team, you will play a vital role in facilitating safe and positive early discharges from acute inpatient wards.
Main Responsibilities
- Collaborate with acute inpatient wards to ensure seamless transitions and effective discharge planning
- Conduct comprehensive assessments of individuals experiencing acute mental health difficulties, acting as gatekeepers to acute admissions
- Provide specialist crisis assessments, focusing on 'hospital at home' alternatives
- Participate in team meetings and contribute to the development of care plans
About the Role
This is an exciting opportunity to work in a fast-paced environment, making a real difference in the lives of our service users. As a Clinical Lead, you will have the autonomy to lead and develop the Early Discharge Team, working closely with colleagues to deliver high-quality care.

Requirements
