Job Title: Team Leader

At Voyage Care, we are seeking a highly skilled and dedicated Team Leader to join our team. As a Team Leader, you will play a vital role in supporting the Service Manager and being the point of contact for the rest of the team when the Service Manager is off-site.

Key Responsibilities:
  • Develop and implement individual support plans, including assessments and relevant risk assessments for new service users.
  • Encourage service users to take an active role in daily living and ensure the appropriate level of individual care and support is identified.
  • Manage the review and maintenance of all support plans to ensure ongoing suitability of support provided.
  • Assist with daily care, including personal care and assisting with bathing, to maintain high standards of hygiene.
  • Participate in and assist with planning and arranging daily outings, holidays, or recreational pursuits to support service users.
  • Build working relationships and engage in positive communication with service users, families, advocates, and professional colleagues to support service users.
  • Undertake and/or assist with household activities, such as cooking, mealtimes, laundering, and cleaning, to promote healthy environments and independent living.
  • Record and complete documentation to ensure accurate and informative service user records, observations, or other documents.
  • Ensure all service users have up-to-date medications and MAR charts, in accordance with the company's Administration of Medication Policy.
  • Maintain compliance and support staff through coordination and completion of supervision, appraisal, and team meetings, and spot checks as per Company policy.
  • Attend, participate, and contribute to relevant multi-disciplinary meetings for service users.
  • Ensure high standards of care and support are delivered through proactive auditing.
  • Organize, attend, participate, and contribute to team meetings to promote communication within the team.
  • Ensure good governance through participation and completion of the auditing process.
  • Ensure staff teams are supported and managed through good leadership and management, paying attention to the completion of supervisions.
  • Undertake all relevant training, including statutory training, to professionally develop and gain skills beneficial to service users.
  • Ensure all policies and procedures applicable to the service and the company are followed and understood to comply with regulations and good practice.
  • Work shifts and visits on a staffing rota, including weekdays, weekends, split shifts, wake nights, sleep-in, and bank holidays as required.

Person Specification:
  • Education/Qualification: Willingness to work towards NVQ in Care & Management, NVQ3/SVQ3 in Care (or equivalent)
  • Competency based interview and assessment
  • Copy of certificates
  • Skills/Abilities: Ability to work as part of a team and without supervision, ability to manage a caseload effectively, understanding of high-quality person-centred care and support, and good communication skills
  • Experience: Experience of working in various settings and with people with different support needs, experience of maintaining accurate basic financial records, and experience of supervising and taking on-going responsibility for a team
  • Knowledge: Knowledge of relevant regulations and legislation, principles of care in the community, and commitment to ordinary life principles
  • Personal attributes: Proactive, flexible, efficient, organized, approachable, reliable, motivated, professional, and effective communicator
