Job Title: Team Leader

At Voyage Care, we are seeking an experienced and dedicated Team Leader to join our team. As a Team Leader, you will play a vital role in supporting the Service Manager and being the point of contact for the rest of the team when the Service Manager is off-site.

Key Responsibilities:
  • Develop and implement individual support plans, including assessments and relevant risk assessments for new service users.
  • Encourage service users to take an active role in daily living and ensure the appropriate level of individual care and support is identified.
  • Manage the review and maintenance of all support plans to ensure ongoing suitability of the support provided.
  • Assist with individuals' care, including personal care and assisting with bathing to maintain a high standard of hygiene.
  • Participate in and assist with planning and arranging individuals' daily outings, holidays, or recreational pursuits.
  • Build working relationships and engage in positive communication with service users, families, advocates, and other professional colleagues.
  • Undertake and/or assist with household activities such as cooking, mealtimes, laundering, and cleaning to promote a healthy environment.
  • Record and complete any necessary documentation to ensure accurate and informative records.
  • Ensure all service users have up-to-date medications and MAR charts, in accordance with the company's Administration of Medication Policy.
  • Maintain compliance and support of staff through coordination and completion of supervision, appraisal, and team meetings.
  • Attend, participate, and contribute to relevant multi-disciplinary meetings for service users.
  • Ensure high standards of care and support are delivered through proactive auditing.
  • Organize, attend, participate, and contribute to team meetings to promote communication within the team.
  • Ensure good governance through participation and completion of the auditing process.
  • Ensure staff teams are supported and managed through good leadership and management.
  • Undertake all relevant training to professionally develop and gain skills beneficial to service users.
  • Ensure all policies and procedures applicable to the service and company are followed and understood.
  • Work shifts and visits on a staffing rota, including weekdays, weekends, split shifts, wake nights, sleep-in, and bank holidays as required.

Requirements:
  • Experience of working in various settings and with people with different support needs.
  • Experience of maintaining accurate basic financial records.
  • Experience of writing or contributing to the writing of detailed support plans.
  • Experience of supervising and taking ongoing responsibility for a team.
  • Experience of mentoring and supporting a staff team.
  • Knowledge of relevant regulations and legislation.
  • Knowledge of support planning and person-centred planning.
  • Knowledge of the principles of care in the community and a commitment to ordinary life principles.
  • Full UK driving licence and own transport.
