About the Role


  • Job Summary: We are looking for a skilled and compassionate Practice Nurse to provide nursing care and support to our patients, working closely with our GP team to deliver excellent patient outcomes.
  • Key Responsibilities:

    • Delivering high-quality patient care, including management of chronic diseases, vaccinations, and health promotion.
    • Working collaboratively with our GP team to provide comprehensive care to patients.
    • Developing and maintaining accurate patient records, ensuring confidentiality and adhering to data protection policies.
    • Participating in audit work, quality improvement initiatives, and continuous professional development to enhance patient care.

Requirements


  • Qualifications: Registered General Nurse, recognized qualification in chronic disease management, and vaccination and immunization training.
  • Experience: Minimum 2 years post-registration experience, with a strong background in primary care and a commitment to patient-centered care.
  • Skills: Excellent communication and interpersonal skills, ability to work autonomously and as part of a team, and a strong commitment to professional development.
