About the Role
We are seeking an experienced and skilled Social Worker to join our Community Mental Health Team as a Care Coordination Specialist. This is an exciting opportunity for a qualified professional to make a positive impact on the lives of individuals experiencing mental health issues.
The successful candidate will work closely with service users, families, and other healthcare professionals to provide comprehensive care coordination services, ensuring that individuals receive seamless and effective support throughout their recovery journey.
Key Responsibilities:
- Liaise with acute inpatient units, crisis teams, primary care services, and voluntary organizations to ensure holistic care delivery.
- Conduct thorough assessments and develop personalized care plans in partnership with service users and their families.
- Coordinate and facilitate access to appropriate health and social care services, promoting independence and self-advocacy.
- Work collaboratively with multidisciplinary teams to ensure effective communication and case management.
Requirements:
- Qualified Social Worker registered with Social Work England.
- Post-qualification experience within mental health services, preferably in care coordination or a related field.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and healthcare professionals.
- Strong assessment and planning skills, with the ability to prioritize tasks effectively and manage multiple cases.
- Able to work independently and as part of a team, with a flexible approach to meet the needs of service users.
Salary and Benefits:
Competitive salary: £38,890 - £44,884 per annum, depending on experience.
