About the Role:

Job Summary

The successful candidate will be responsible for planning and coordinating holistic care solutions for individuals over 65 with complex needs and conditions. You will work closely with health, social care, mental health, and voluntary sector services to deliver comprehensive care solutions that meet patients' needs.

Key Responsibilities:
  • Planning and coordinating care plans across multiple organizations
  • Collaborating with professionals, patients, and carers to design personalized care solutions
  • Ensuring seamless transitions between services
Requirements:

To succeed in this role, you will need:

  • A degree in a relevant field (e.g., social work, nursing)
  • Experience working in healthcare or social care, preferably in a senior role
  • Knowledge of the Care Act, Mental Capacity Act, and adult safeguarding policies
What We Offer:

In return for your expertise and dedication, we offer a competitive salary of £60,000 - £80,000 per annum, depending on experience. You will also benefit from ongoing training and development opportunities to enhance your skills and career progression.
