Job Summary

We are seeking a highly motivated and compassionate individual to join our team as an Intermediate Care Enabler. As a key member of our multidisciplinary team, you will play a vital role in supporting patients to regain their independence and achieve their goals.

Key Responsibilities
  • Build professional relationships with service users, ensuring they are aware of and engaged with their care and support.
  • Support members of the team in achieving agreed standards of care.
  • Ensure patients' needs are being met as identified in their care plan.
  • Undertake therapy tasks with patients independently, using their care plan and goals set by the therapy team.
  • Use empathy, sensitivity, and good interpersonal skills to encourage patients and their carers in an active approach to regaining and maintaining personal independence.
Requirements
  • Good literacy and numeracy skills.
  • Basic IT skills.
  • Care Certificate/NVQ Level 2 in Health and/or Social Care.
  • Ability to work within a team.
  • Able to work independently.
  • Ability to manage own time.
  • High-quality organisational skills.
Desirable
  • Previous experience as an enabler.
  • Previous experience in a social care setting.
