Job Title: Intermediate Care Enabler

We are seeking a highly motivated and compassionate Intermediate Care Enabler to join our team at Medway Community Healthcare NHS Foundation Trust. As an Intermediate Care Enabler, you will play a vital role in supporting patients to regain their independence and achieve their goals.

About the Role:

As an Intermediate Care Enabler, you will work closely with a multidisciplinary team to provide high-quality care and support to patients in their own homes or in our community settings.

Key Responsibilities:
  • Support patients to regain their independence and achieve their goals through a person-centred approach.
  • Work collaboratively with a multidisciplinary team to provide high-quality care and support to patients.
  • Develop and implement individualised care plans to meet the unique needs of each patient.
  • Provide education and support to patients and their families to promote independence and self-management.
  • Contribute to the development and implementation of policies and procedures to ensure high-quality care and service delivery.
Requirements:
  • Good literacy and numeracy skills.
  • Basic IT skills.
  • Care Certificate/NVQ Level 2 in Health and/or Social Care.
  • Ability to work within a team and independently.
  • High-quality organisational skills.
