About the Role

As a Community Support Specialist, you will play a vital role in connecting with individuals, families, and carers to understand their needs and provide support. You will work closely with our commissioning team to enable individuals to access the support they require, whether it's connecting with their local community, referring them to other agencies, or providing direct support to meet their unmet needs.

Key Responsibilities

  • Conduct assessments to understand individual needs and develop personalized support plans
  • Work collaboratively with our commissioning team to ensure seamless support delivery
  • Build effective relationships with individuals, families, and carers to understand their needs and preferences
  • Identify and report potential safeguarding concerns, mental capacity issues, and other high-risk situations
  • Manage a caseload and work towards clear objectives

About You

To be successful in this role, you will need to have:

  • A Level 2 qualification in Maths and English
  • Relevant experience working with adults in a social care setting, including knowledge of supported living and tenancies
  • Understanding of social care organizations and their roles and responsibilities
  • Excellent communication and interpersonal skills, with the ability to build effective relationships
  • Ability to recognize the limits of your accountability and responsibility, and seek support when needed
