Job Summary

We are seeking a highly skilled and experienced Clinical Pharmacist to join our dynamic team at Archwood Medical Practice. As a key member of our clinical team, you will play a vital role in optimising prescribing, improving patient care, and managing long-term conditions.

Main Responsibilities
  1. Conduct medication reviews with patients, providing education, prescribing, and monitoring as per guidelines and professional competencies.
  2. Proactively manage patients with complex polypharmacy, implementing own prescribing changes and ordering relevant monitoring tests.
  3. Lead the implementation and management of a robust medication review process.
  4. Collaborate with GPs, practice staff, and healthcare teams to answer medication-related enquiries, suggesting and recommending solutions.
  5. Update patient medication records post-hospital discharge and outpatient appointments, advising GPs of any issues that may arise.
  6. Provide leadership to the general practice team to ensure compliance with CQC standards regarding medicines, undertaking risk assessments and management, and ensuring compliance with medicines legislation.
  7. Influence and implement changes promoting safe and cost-effective prescribing.
  8. Ensure all MHRA drug alerts are actioned and recorded appropriately.
  9. Work alongside the PCN Clinical Pharmacist team, implementing their work into practice.
  10. Contribute to attainment of prescribing targets, including Quality Outcome Framework (QOF) and Investment and Impact Fund (IIF).
  11. Manage time effectively to ensure all targets are met.
  12. Complete or implement clinical audit work to improve prescribing safety and patient care.
  13. Take an active role in supporting practice initiatives.
Person Specification

Qualifications

  • Registered with the relevant pharmacy regulator (GPhC).
  • Degree in pharmacy, MPharm/BPharm.
  • Independent prescribing qualification.

Desirable

  • Postgraduate diploma in Clinical Pharmacy.
  • Experience working in General Practice.
  • Clinical Systems trained (EMIS, Docman).

Experience

  • Experience working with GPs.
  • Experience of General Practice and understanding of medicines optimisation roles.
  • Experience of patient consultation.
