About the Role
We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Archwood Medical Practice. As a key member of our clinical team, you will play a vital role in optimizing prescribing, improving patient care, and managing long-term conditions.
Key Responsibilities
As a Clinical Pharmacist, you will be responsible for:
- Undertaking medication reviews with patients face-to-face and over the phone, including patient education, prescribing, and monitoring as per guidelines and within professional competencies.
- Proactively managing patients with complex polypharmacy, implementing own prescribing changes (as an independent prescriber) and ordering relevant monitoring tests.
- Leading on the implementation and management of a robust medication review process.
- Working closely with GPs, other practice staff, and healthcare teams to answer all medication-related enquiries, suggesting and recommending solutions.
- Updating patient medication records post-hospital discharge and outpatient appointments and advising GP of all issues that may arise.
- Providing leadership to the general practice team to ensure the practice is compliant with CQC standards where medicines are involved, undertaking risk assessment and management, and ensuring compliance with medicines legislation.
- Influencing and implementing changes promoting safe and cost-effective prescribing.
- Ensuring all MHRA drug alerts are actioned and recorded appropriately.
- Working alongside PCN Clinical Pharmacist team and implementing work initiated by them into practice.
- Contributing to attainment of prescribing targets, including Quality Outcome Framework (QOF) and Investment and Impact Fund (IIF).
- Managing time effectively to ensure all targets are met.
- Completing or implementing clinical audit work to improve prescribing safety and improve patient care.
- Taking an active role to support practice initiatives.
Person Specification
We are looking for a candidate with:
- Registration with the relevant pharmacy regulator (GPhC).
- A degree in pharmacy, MPharm/BPharm.
- An independent prescribing qualification.
- Post-graduate diploma in Clinical Pharmacy (desirable).
- Experience working in General Practice and understanding of medicines optimisation roles.
- Experience of patient consultation.

Archwood Medical Practice is a well-established, family-friendly practice with a CQC rating of Good. We care for circa 7900 patients in the Woodley, Romiley, and Bredbury areas of Stockport. We work closely with our Primary Care Network (PCN) and local community services colleagues to provide excellent healthcare to our patients.

Woodley Health Centre is a modern purpose-built building with excellent facilities for staff and patients, including free on-site parking. Close to junction 25 on the M60, Woodley has excellent road and public transport links into Manchester City Centre and a short drive from the beautiful Peak District.
