Senior Hospice Nurse

This is a unique opportunity to work in our award-winning Hospice Community Services (HCS), which offers innovative models of care that work collaboratively with other providers. We are currently reviewing our services to align with local community providers, making this an exciting time to be part of our team.
We assess needs and plan, implement, and deliver tailored packages of care that meet individual needs. Our goal is to provide seamless support to patients, families, and carers in the community.
Main duties of the jobYou will be a vital part of a supportive and specialist team that has a great reputation for ensuring patients, families, and carers receive the support they need. In this role, you will work flexibly to support and deliver care, undertaking triage and rapid response visits, managing palliative discharges, assessing and reviewing care support, conducting full patient assessments, and planning care.
You will need to have relevant palliative care experience to be successful in this role and will require a driver's license and use of a vehicle to work in the community.

Job responsibilitiesOther responsibilities include:
- To use telephone triage to assess patient and family need, providing advice and support.
- To offer face-to-face assessment, providing the best and appropriate care for patients at the end of life that meets their choice and supports their families and carers.
- To assess and review patient needs in any setting, including home, hospital, care home, and Hospice.
- To be aware of responsibilities and processes in safeguarding vulnerable adults and children.
- To promote personal and professional development of all nursing staff.
- Registered Nurse Qualification and NMC pin number.
- A commitment to work in Palliative Care Nursing.
- Clinical Assessment and Diagnosis Skills.
- Community nursing experience.
- Excellent written and verbal communication skills.
- Flexibility and willingness to work on rotating shift patterns.
- Advanced Non-Medical Prescribing desirable but not essential.
