About the Role
We are seeking a highly skilled and compassionate Palliative Care Specialist to join our team at St Luke's Hospice. As a key member of our Community Services, you will play a vital role in delivering high-quality care to patients and their families in the community.
Key Responsibilities
- Assess patient needs and develop personalized care plans to meet their unique requirements.
- Provide expert clinical guidance and support to patients, families, and carers.
- Work collaboratively with multidisciplinary teams to ensure seamless care delivery.
- Conduct thorough patient assessments and reviews to ensure optimal care outcomes.
- Develop and maintain strong relationships with patients, families, and carers to ensure their needs are met.
About You
We are looking for a registered nurse with a strong background in palliative care and community nursing. You will have excellent clinical assessment and diagnosis skills, as well as the ability to work independently and as part of a team. A commitment to delivering high-quality care and a passion for working with patients and families are essential.
