Special Education Needs and Disability Support Professional

As a Send Keyworker at Service Care Solutions in Stoke, Staffordshire, you will play a vital role in supporting adults and children with special educational needs and disabilities (SEND).
Job DescriptionYou will work on a 3-month contract, coordinating support from qualified social work, Educational Psychology, Health, and other professionals as needed.
The successful candidate will be responsible for creating assessments of need and developing education, health & care plans that align with relevant legislation and the Code of Practice.
You will build inclusive relationships with individuals with SEND and their families, respecting diversity and supporting choice and independence to achieve realistic outcomes.
Salary: £25,000 - £30,000 per annum
Key Skills and Qualifications- A significant experience in planning personalised support for individuals with disabilities, promoting their independence and tailored progress.
- In-depth knowledge of disabilities, special educational needs (SEN), and their impact on individuals, families, and society.
- Skilled in empowering individuals and families to plan for immediate, medium, and long-term outcomes with dynamic options.
- Proficient in using IT for documentation, data management, communication, and reporting.
- Knowledge of safeguarding, abuse prevention, and relevant legislation including the Mental Capacity Act, Children Act, and SEN Code of Practice.
