About the Team

We are seeking a skilled and motivated individual to join our Crisis and Home Treatment Team as a Multidisciplinary Crisis Response Team Member.

This is an exciting opportunity to work as part of a multidisciplinary team providing crisis and home treatment services for individuals aged 17 and over.

As a member of our team, you will be responsible for delivering high-quality care and support to patients in their homes or in community settings.

You will use your initiative and take responsibility for ensuring the quality of your work and the service provided to patients.

Key Responsibilities
  • Operate as part of a multidisciplinary team providing crisis and home treatment services for individuals aged 17 and over.
  • Use initiative and take responsibility for ensuring the quality of your work and the service provided to patients.
  • Proactively engage with partnership agencies and students, undertaking full psycho-social assessments and ensuring thorough documentation and dissemination of information in line with Trust policy and procedure.
