About the Position

We are seeking a skilled Disability Care Coordinator to join our team at Merri Health, coordinating high-quality disability care services to support individuals in Victoria.

The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a diverse environment.

Key Responsibilities:

  • To coordinate disability care services, ensuring that individuals receive the support they need to live independently.
  • To develop and implement individualized care plans, working collaboratively with carers, families, and healthcare professionals.
  • To maintain accurate records and reports, adhering to the Intake and Registration Operation Manual.

Requirements:

  • A Certificate IV in Disability, Community Care or equivalent is essential.
  • Knowledge of the community care sector is highly desirable.
  • Proven ability to foster and maintain relationships is essential.
