Rural Clinical Pharmacist Specialist

About the Role
We are seeking an experienced Clinical Pharmacist to join our team in North Oxfordshire Rural Alliance Primary Care Network (PCN). As a key member of our pharmacy team, you will provide comprehensive clinical pharmacy services to patients across our network of five rural surgeries.
As a Rural Clinical Pharmacist Specialist, you will be responsible for leading on medicines optimisation initiatives, conducting clinical medication reviews, and implementing changes to improve patient outcomes. You will also work closely with our Senior Clinical Pharmacist and other healthcare professionals to ensure seamless integration between primary and secondary care.
The ideal candidate will have experience working in general practice, excellent communication skills, and a passion for improving patient care. If you are a motivated and proactive pharmacist looking to make a real difference in your community, we encourage you to apply.
Responsibilities
- Lead on medicines optimisation initiatives across the PCN
- Conduct clinical medication reviews and implement changes to improve patient outcomes
- Work closely with the Senior Clinical Pharmacist and other healthcare professionals to ensure seamless integration between primary and secondary care
- Develop and maintain strong relationships with GPs, nurses, and other healthcare professionals to promote collaborative working and shared decision-making
Requirements
- Registered Pharmacist with the General Pharmaceutical Council
- Experience working in general practice or a similar setting
- Excellent communication and interpersonal skills
- Able to work autonomously and as part of a team
Salary: £55,000 - £65,000 per annum, depending on experience
