Integrated Care Team Member

Job Description
We are looking for a highly motivated and organised Integrated Care Team Member to join our team at the Lincolnshire Community Health Service NHS Trust. In this role, you will be responsible for delivering person-centred care and assisting healthcare professionals with patient interventions.
Your duties will include working closely with Occupational Therapists, Physiotherapists, Nurses, Paramedics, and other healthcare professionals to deliver clinical interventions defined within individualised patient care plans. You will also maintain high-quality standards in all aspects of care delivery and work in partnership with primary care colleagues, patients, carers, and statutory, independent, and voluntary organisations.
The successful candidate will have an estimated salary of £25,000 - £30,000 per annum and will be required to work 7 days a week, 365 days a year, as part of our comprehensive support system for patients.
