We are seeking an experienced Registered Nurse (Adult) to join our team as a Band 5 Community Care Coordinator at Unity Healthcare Recruitment Ltd. This rewarding opportunity offers the chance to work in a variety of community settings, delivering high-quality nursing care and promoting independence.

This role involves working collaboratively with other healthcare professionals, such as GPs, therapists, social workers, and voluntary sector partners, to ensure coordinated care. Key Responsibilities:

  • Clinical Care
    • Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
    • Conduct holistic assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
    • Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
    • Provide health education and advice to patients and carers to support self-management and promote independence.
  • Community-Based Care
    • Work autonomously in patients' homes and community settings, prioritising tasks and responding to changes in patients conditions.
    • Manage a daily caseload, using digital tools and systems to plan, record, and report on care delivery in line with organisational policies.
  • Team Collaboration
    • Participate in multidisciplinary team meetings and case conferences to support care planning and review.
    • Act as a patient advocate, ensuring their preferences and needs are central to care planning and decision-making.

The ideal candidate will have clinical experience in a relevant setting and strong clinical assessment and decision-making skills. A competitive salary of £26-31 per hour and flexible working hours are offered.
