Job Overview

We are seeking a skilled Community Care Specialist to join our Integrated Urgent Community Response and NHS@Home services in Swindon. This role involves delivering high-quality nursing care to patients in their own homes, promoting independence and dignity.

The successful candidate will work closely with the GP, multidisciplinary teams, and social care services to develop strong working relationships and ensure effective care planning.

Key Responsibilities
  • Care Delivery: Provide effective nursing care to patients in their own homes, ensuring safe and high-quality practice.
  • Collaboration: Work collaboratively with community nursing day and night services, specialist community services, and remote monitoring systems like Doccla.
  • Leadership: Contribute to leadership, management, and organisation of the service team, ensuring evidence-based and clinically effective standards.
Requirements
  • RN (Adult) first or second level registration with the NMC.
  • Evidence of continuing professional development relevant for Band 5 post.
  • Extended skills such as Intravenous therapy, catheterisation, phlebotomy.
Salary and Benefits

The salary range is £29,970 to £36,483 per year, pro rata. As an NHS employee, you will have access to various benefits, including pension schemes, flexible working options, and opportunities for career progression.
