About the Role

We are seeking a skilled and compassionate Registered General Nurse to join our community nursing team in Somerset. As a District Nurse Specialist, you will play a vital role in delivering high-quality patient care in the comfort of patients' own homes.

Key Responsibilities

  • Work closely with the Team Leader to coordinate community nursing teams and ensure effective workload management.
  • Develop and maintain strong relationships with patients, families, and other healthcare professionals to provide holistic care.
  • Conduct thorough assessments, develop care plans, and implement evidence-based interventions to promote patient independence and empowerment.
  • Collaborate with other healthcare agencies to ensure seamless care transitions and optimal patient outcomes.

Requirements

  • Registered General Nurse with current NMC registration.
  • Level One Registration with appropriate registration with the NMC as per post requirements.
  • Desirable: Teaching/Assessing Qualification, experience of working in multi-agency services, and evidence of Continuing Professional Development.
