About the Role

We are seeking a skilled and compassionate District Nurse to join our team at Somerset NHS Foundation Trust. As a District Nurse, you will play a vital role in providing high-quality patient care in the community, working closely with other healthcare professionals to deliver safe and effective care.

Key Responsibilities
  • Work collaboratively with the community nursing team to coordinate care and meet the needs of patients in the community.
  • Develop and implement holistic care plans that address the physical, emotional, and social needs of patients.
  • Provide expert wound management and palliative care to patients in their own homes.
  • Work in partnership with other healthcare professionals and agencies to ensure seamless care delivery.
Requirements
  • Registered General Nurse with current registration with the NMC.
  • Level One Registration with appropriate registration with the NMC as per post requirements.
  • Teaching or assessing qualification desirable.
