Job Summary

We are seeking a skilled and compassionate Registered General Nurse to join our community nursing team at Somerset NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality patient care in the comfort of patients' own homes.

Main Responsibilities
  • Work closely with the Team Leader to coordinate community nursing teams and ensure effective workload management.
  • Collaborate with other healthcare professionals and agencies to provide comprehensive patient care.
  • Encourage client independence and empowerment through holistic assessment and care planning.
  • Develop and maintain knowledge base in line with the Knowledge and Skills Framework (KSF) requirements.
Requirements
  • Registered General Nurse with Level One Registration with the Nursing and Midwifery Council (NMC).
  • Desirable: Teaching or Assessing Qualification, experience of working in multi-agency services, and evidence of Continuing Professional Development.
