Job Summary

We are seeking a motivated and compassionate Care Worker to join our care at home team. As a Care Worker, you will play a vital role in supporting individuals with a range of disabilities to maintain their independence while living at home.

Main Responsibilities
  • Provide person-centered support to service users, promoting their physical, emotional, and social well-being.
  • Assist with daily living tasks, such as personal care, medication management, and household chores.
  • Develop and maintain social networks and relationships with service users, promoting their social inclusion and community engagement.
  • Support service users in developing skills and knowledge to live as independently as possible, adhering to risk assessment processes.
  • Participate in the development, implementation, and review of personal plans with service users.
  • Provide emotional support and guidance to service users during significant life changes or losses.
  • Continuously develop your knowledge and practice through participation in training and development activities.
Requirements
  • Experience in a care setting, preferably in a similar role.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Willingness to participate in ongoing training and development.
