Primary Care Mental Health Nurse Opportunity

Service Care Solutions Ltd is seeking a skilled Mental Health Nurse to join our team in a GP Practice setting in Market Drayton. As a Primary Care Mental Health Nurse, you will play a vital role in providing high-quality care to patients with mental health needs.

Key Responsibilities:
  • Conduct triage and assessments of mental health needs using evidence-based outcome measures
  • Work in partnership with service users to identify effective strategies for managing mental health problems
  • Provide information, advice, education, and health promotion to service users, carers, and the wider community
  • Critically analyze service user information and formulate individual plans of care using evidence-based brief interventions
Requirements:
  • Must have GP Practice/Primary Care experience and knowledge of using EMIS
