Primary Care Mental Health Nurse Opportunity

We are seeking a skilled Mental Health Nurse to join our team at a GP Practice in Market Drayton. As a Primary Care Mental Health Nurse, you will play a vital role in providing triage and assessments of mental health needs, working in partnership with service users to identify effective strategies for managing their mental health.

Key Responsibilities:

  • Provide triage and assessments of mental health needs using evidence-based outcome measures
  • Work in partnership with service users to identify effective strategies for managing their mental health
  • Provide information, advice, education, and health promotion to service users, carers, and colleagues
  • Critically analyse service user information and formulate individual plans of care using evidence-based brief interventions

Requirements:

  • Must have GP Practice/Primary Care experience and knowledge of using EMIS
