We are seeking a highly skilled and experienced Nurse Clinician for Chronic Disease Management to join our team. The successful candidate will work closely with our clinical team to develop and implement effective care programs for patients with long-term conditions.

**Key Responsibilities:**

  • Developing, implementing, and integrating effective care programs for patients with long-term conditions.
  • Conducting patient assessments to ensure healthcare provision is appropriate and evidence-based.
  • Creating, implementing, and embedding health promotion and wellness programs.

**Requirements:**

  • Registered Nurse (Nursing and Midwifery Council) qualification.
  • Experience working in primary care environments.
  • Excellent communication skills.
  • Prioritizing health concerns and intervening when appropriate.
