AppLocum is seeking a skilled Practice Nurse to join our team of healthcare professionals in GP surgeries across Cheshire. As a Practice Nurse, you will have the opportunity to work in a dynamic and supportive environment, providing high-quality care to patients with chronic conditions.

About the Job:

  • Chronic Disease Management: Develop and implement care plans for patients with asthma, diabetes, hypertension, and COPD.
  • Health Screenings: Conduct routine health checks, including blood pressure monitoring, cervical smears, and cholesterol checks.
  • Vaccinations and Immunisations: Administer routine and travel vaccinations, as well as flu immunisations.
  • Health Promotion: Provide advice on lifestyle, diet, and exercise to promote patient well-being.
  • Patient Documentation: Maintain accurate records of patient details, consultations, and treatments.

Key Skills and Qualifications:

  • NMC Registration: Valid PIN with full registration.
  • Experience: Minimum of 6 months in a primary care setting.
  • Strong clinical knowledge and experience in chronic disease management, immunisations, and general practice care.
  • Up-to-date mandatory training, valid DBS, and indemnity cover.
